Accurate dosage calculation is one of the most critical skills for nurses, pharmacists, and medical professionals. A simple mathematical error—such as misplacing a decimal point or failing to convert units correctly—can lead to severe underdosing or a potentially fatal overdose.

The Basic Formula (Desired Over Have)

The most widely used formula in nursing pharmacology is the "Desired over Have" method, also known as the formula method. It is used when the doctor's order differs from the concentration of the medication supplied by the pharmacy.

  • D (Desired): The dose the provider ordered (e.g., 500 mg).
  • H (Have): The strength of the drug on hand (e.g., 250 mg).
  • Q (Quantity/Vehicle): The volume or unit that contains the available dose (e.g., 1 tablet, or 5 mL of liquid).
  • CRITICAL RULE: The units for "Desired" and "Have" MUST match before dividing. If you want 1g and have 500mg, you must convert 1g to 1000mg first.

Weight-Based Dosing

Weight-based dosing is standard practice in pediatrics, veterinary medicine, and critical care. Because patient sizes vary drastically, medications are often prescribed as a specific amount of drug per kilogram of body weight.

  1. Convert Weight: Always convert the patient's weight from pounds (lbs) to kilograms (kg) by dividing by 2.2.
  2. Calculate Total Dose: Multiply the weight in kg by the ordered dose (e.g., mg/kg).
  3. Divide if Necessary: If the order says "divided in two equal doses (BID)", divide the total daily dose by 2 to find the single administration amount.

IV Flow Rate & Drip Rate Calculations

When administering IV fluids or medications via gravity (without an electronic IV pump), the nurse must calculate the drip rate in drops per minute (gtt/min).

The Drop Factor

The drop factor is determined by the specific IV tubing used. Macrodrip tubing usually delivers 10, 15, or 20 drops per mL (used for rapid infusions). Microdrip tubing always delivers 60 drops per mL (used for pediatrics or sensitive medications).

The Formula

(Volume (mL) ÷ Time (min)) × Drop Factor

Always ensure your time is converted to minutes before calculating. For electronic IV pumps, the calculation is simpler: Total Volume (mL) ÷ Total Time (Hours) = mL/hr.
